Please make sure to complete your intake paperwork before our first session. It is a smooth and secure process that you can complete online.
If you prefer, you can spend a part of your first session filling them out with me. Thank you in advance!

Limited metered parking.
Payment
You can pay by credit card, check, or cash. If using a credit card, your information will be securely stored in my practice management system.
As a healthcare provider, I also accept Health Savings Accounts (HSA) and Flexible Spending Accounts (FSA).
